Good day and thank you for your time and consideration of this urgent Hospice need in the highly desirable Seattle, Washington.My client is a very large and well respected healthcare organization in the Pacific Northwest and they are in need of a board certified Nurse Practitioner for locum tenens Hospice visits who can start as soon as credentialed. - Immediate start date. Full time, ongoing for several months with chance for extension. - Practice location but home visits will be from many locations in Seattle, WA - NP will see hospice patients in their home to determine eligibility for recertification for hospice and will frequently discuss patient eligibility with the RN Case Manager. - Oncology and Cardiac symptom management is preferred. - Opioid prescribing for hospice patients is required. - Schedule is 8a – 5p M-F, call is 1 week month. - 5 – 6 patients per day w/ 2 add-ons. - Staff is RN- Case Managers 2 /team, MD 1 /Team, MSW 1/Team, Chaplain.
