Description
Are you looking for a new and exciting position working in healthcare? A growing Patient Centered Medical Home, (PCMH) model clinic in downtown Philadelphia is seeking a highly motivated, compassionate, and dynamic clinical professional Full Time, Nurse Practitioner. Clinical NP and "Same Day" (similar to urgent care) providers needed. The Nurse Practitioner will focus on the management and improvement of health for patients with multiple chronic medical conditions.be is responsible for a broad range of skills, clinical expertise and developing a plan of care for the member, in conjunction with the providers and the whole multi-disciplinary team. The Clinical Nurse Practitioner will be responsible for a panel of 200-250 chronic care ambulatory patients in the clinic. They will be responsible for working interdependently with other professionals to develop a strong care plan. Education & Experiences:
- Licensed Nurse Practitioner in Pennsylvania
- At least 2 years NP experience after training
- Experience with patients who have multiple chronic medical conditions, i.e. CHF, COPD/Asthma, HTN, Diabetes, Chronic Renal Failure
- ACLS certification is a plus
- Knowledge of managed care and insurance processes is preferred.
- Experience with the underserved population is preferred
- Experience with using EMR and efficiency a must in completing EMR notes within 48 hours, PrimeSuite preferred Benefits
