Dates of Coverage: As soon as credentialed and on going
Details/Requirements for Assignment #103995:
-- Active Wisconsin license required or IMLC
-- Board Certification required, will accept truly Board Eligible
-- Outpatient Procedures Monday – Friday with 24 hour weekend call
-- Bread and Butter, Advanced procedures not required
-- Ongoing coverage need, willing to take multiple providers
-- Flexible Schedule, 7 on 7 off, Monday – Friday, weekend call coverage
-- 24 hour call schedule, flexible – Friday 7am or 5pm start to Monday 7am
