• Credentialing Time Frame: 90 days
• Types of cases: General, Ortho, GI-Endo, Eyes, Healthy Peds, Urology, Robotics, OB/GYN, Bariatrics, Vascular, cardiac ablations, interventional radiology, neuro-interventional radiology, neuro, bronchoscopy. No trauma and cardiac done by cardiac MD team
• Avg # of monthly cases: 1,800
• Avg monthly OB Delivery: 325
• EMR: Meditech
• Care Team Model: 25 CRNA; 19 MD
• Shifts: Monday - Friday either five 8 or or four 10 hour shifts. Approx 7a- 3pm 7a- 5pm; Weekends and call optional
• No Providers that worked at this site or affiliated site that is within 75 miles of this site  
• Must have FL license
• Must be Board Certified
• Must have clean file and background
• Must have Full-time Open Availability 
• $500 referral bonus received for any referral who works 30 days.
