• Join One of the Largest & Most Successful P.A.C.E. Programs in the Country
? Drive The Mission / Oversee 4 Clinics in South and East Los Angeles Area
? Responsibilities: Manage 2-3 PCP Per Site / 40% Clinical / 60% Administrative / NextGen EMR
• P.A.C.E. Provides: 2 Meals Per Day For Patients / Roundtrip Transportation to Facility / Social & Interactive Activities / Physical, Occupational & Aqua Therapy
? Onsite: Behavioral Health / Podiatry / Acupuncture / Pharmacy / Blood Lab / X-Ray / Dentistry
? Support Staff: LVN / RN / Registered Dietitian / Social Worker / Medical Assistants / Center Manager
? Regular Individual Comprehensive Patient Review by Interdisciplinary Team (IDT) - Comprised of PCP, Nursing Staff, Therapists, Nutritionists & Social Workers
? Program Includes At Home Care for Qualifying Patients
• Work Schedule: 40 Hours Per Week / M-F 8am – 5pm with No Weekend Coverage!
? Minimal Call
? 5-7 Patients Per Day
? Total Patient Panel of 100 – 120
? Patient Follow-up Every 6 Weeks
• Employed Salary: $240K – $260K Based Upon Years of Experience + Year End Bonus
