Description
General Responsibilities: • Work closely with senior management team members to help develop and guide strategic direction and tactical plans for assigned communities. • Develop positive relationships on behalf of the facility and Diakon Lutheran Social Ministries with physicians, hospitals and clinics • Promote a learning culture by teaching at the bedside, giving regular in-services and ensuring education is an integral part of the mission • Remain current on long-term care regulations and compliance issues • Provide guidance and opinions on current clinical practices, policies and procedures, as well as assist with the evaluation of new clinical programs, policies and procedures • Provide education and support for the integration of hospice into the facility culture • Be a mentor and role model for community physicians in demonstrating skilled and compassionate care and supporting collaborative partners • Perform timely and detailed new-resident physical examinations and resident discharges • Stay current on medical and clinical issues related to geriatrics, pain management, hospice, palliative care and long-term care • Partner with consultant pharmacists – using data and experience to identify, evaluate, and produce solutions for medication-related uses at the community • Partner with the NHA/ED and DON to ensure that patients have adequate, competent and appropriate physician services and that attending physicians meet performance expectations • Review incident and accident reports to help identify hazards to the health and safety of patients and staff • Meet with, review and respond to federal, state, local and other external surveyors and inspectors • Act as a liaison between attending physicians and other health-care professionals serving the patient • Be responsive to nursing staff concerns as well as resident and family relationships and needs • Assist with recruitment, interviewing, hiring, oversight and mentoring of new physicians and education of attending physicians • Participate in weekly, monthly, quarterly and annual meetings/conference calls.
