Description
Interdisciplinary teams collaboratively manage complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members.

The Urgent Care Provider primarily provides medical care and management to patients who request urgent visits to the NCM on call from 8:00 AM to 5:00 PM Monday – Friday (Non-Holiday). This position prescribes appropriate diagnostics and interventions to avoid unnecessary transfers and acute admissions. Communicates and collaborates with the assigned primary provider and interdisciplinary team in achieving patient’s care goals.

Responsibilities

-Following the visit promptly communicates with the PCP and specialist as needed along with communicating findings to their primary assigned provider.

-Manage patient care with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizations.

-Diagnose, prescribe and treat within the scope of the Provider and consistent with standards of care.

-Participate in regular management/interdisciplinary team meetings to coordinate and collaborate on patient care needs.

-Completes the required documentation in a timely manner allowing the care team access to the care provided and a note to be generated to the PCP.

-Participate in a rotated on-call program for home-based patient support after-hours.

“Must-Have” Skills for the job:

1. Current unrestricted state license and current state driver's license

2. A minimum of 2 years' experience as an NP or PA

3. Completion of Nurse Practitioner or Physician Assistant accredited program

4. Knowledge of CPT, ICD-9 and HCPC codes

5. Knowledge of clinical standards of care

6. Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful

7. Passion for geriatric patient care

8. Comfortable with conducting house calls

What you need to know

1. Since they are responding to urgent needs and not regularly scheduled visits, the Urgentivist's commute may vary day to day

2. May not have as many visits as a regular provider does

3. Urgent Care Provider needs to have bandwidth to be the go to person who gets called and can arrive immediately to a location.
