Springfield, Mass area: We invite an experienced NP or PA to join an important healthcare team providing in-home healthcare visits, consultations and treatment. This is a traveling role that will take you around western Massachusetts – a territory that includes Worcester County, and extends south to the CT border, and west to the edge of the Berkshires.

Your important job is to provide occasional, short-term primary care and case management visits with people who are on Medicaid or/or Medicaid -seniors, and persons of varying ages with disabilities, with an emphasis on geriatrics.

Some Spanish language skills are necessary – candidates who are fluent will receive priority attention!

Patient visits are triggered by an acute event, or a significant change in their condition, which necessitates a follow up with a skilled Advanced Practice Provider. As part of a Rapid Response Team, you may encounter urgent care and/or behavioral health issues. Your visit is intended to prevent the need for the patient to have an ER visit or hospitalization. Some visits are post-discharge follow ups. You will take detailed histories, perform physical exams, order appropriate studies and tests, make a definitive diagnosis, develop a treatment plan, and perform any required follow up. Timely, thorough documentation is a key component of this role. You will report to a clinical Medical Director.

You will have a full time weekday schedule (M-F, 8:30-5) and will share a weekend rotation about 8 times/year (when your week will include two weekdays plus Sat & Sun).
