Internal Medicine, Family/Internal Medicine Physician Opportunity
JOB SUMMARY:
Physician is responsible for providing comprehensive health care to both well and ill patients and/or family members; promotes wellness, prevention of health problems, and maintenance of current health; intervenes to care for chronic and acute illness.
EDUCATION AND REQUIRED EXPERIENCE:
Must be a MD/DO , Board Certified/Eligible.
Schedule: Physicians are scheduled to see return patients either 15/30 or 20/40 Schedule, 9 half days, and 1- day as admin time. [Half day assigned by the Medical Director] Standard Schedule (8-12 a.m.; 1-5 p.m.) Monday-Friday. Electronic Medical Record; About 1 in 12 weeknights (Mon.-Fri.) Call is taken from home, assisted by Nurse Advice Unit which screens all incoming calls when out-patient offices are closed, and 3 Urgent Care Centers (open 7 a.m. to 11 p.m. 365 days a year). The on-call physician simply triages calls from the Nurse Advice Unit (typically 3-6 per night), but does not go to the Urgent Care or hospital. During weekdays office hours there is Doctor of the Day in-office call rotated among all physicians at that office, to respond to refill requests, SNF calls, urgent lab results, etc. for providers out of the office. Hospitalists do all admissions 24/7. About 1 in 12 weekends (Saturday & Sunday) you receive calls from the Nurse Advice unit as above.
All primary care physicians are requested to admit to three local SNFs on a rotating basis. The average TOTAL SNF census per physician is only 2-4 patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES
(Duties include the following, other duties may be assigned.) • Review patient records to determine health status. • Perform physical exams including health history. • Document appropriate information and/or health appraisals given by patients and family members. • Assess patient clinical problems and patient health care needs. Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with physician and other health professionals to prepare and implement comprehensive patient care plan. Co-manages patients of the supervising physician by sharing visits. • Assess psychosocial, cognitive and personal assets and limitations of patient and family members; plan care to address these needs. • Orders, interprets, and evaluates appropriate diagnostic tests to identify and assess patient's clinical problems and health care needs. This includes laboratory, radiology, and other diagnostic modalities. Follows up on results of all ordered diagnostic testing.
