DESCRIPTION:  
Outpatient/Primary Care for Employees and Family at an on-site clinic for a business
Patients: Avg. 10-15 per day, ages 2 and up
Procedures: Suturing, small mole removals and skin biopsies required
MUST have 2 years of Workers Comp experience! (required, will reject if provider does not have)
MUST have 3 years of Primary Care experience!
Must be comfortable working independently in the case MA is out
Dates: 9/4, 10/15, 11/25, 11/27, 12/24, 12/28,12/30&31
  - Mon 7:00 am-6:00 pm
  - Wed  8:00 am-6:00 pm
  - Thu at 600 am -6:00 pm (working two different sites)
  - Fri at  7:30 am-11:30 am

REQUIREMENTS:
DEA 
BLS
Prefer local candidates due to sporadic days
OH State License
