Join a group of 8 Physicians including Internal Medicine, Family Practice and Psychiatry plus 6 Midlevels. A growing medical group practice providing inpatient and outpatient services to patients on the North side and West suburbs of Chicago.
Geriatric Focus
Group has privileges at 10 acute care Hospitals, 35 SNFs, 3 LTACHS, 3 Psychiatric hospitals and several Assisted Living Facilities.
Rounding on both inpatients in acute care hospitals and providing medical care to inpatient mental health patients.
Position Responsibility Outline:
Full time 40 hours per week (7 to 9 hour work day) Monday through Friday,no weekends
75% assignment to admitting, rounding on and discharging inpatients at affiliated SNFs and 25% assignment to rounding on inpatients admitted to hospitals. (Primarily a SNF Hospitalist role)
Participation in developing and providing medical coverage of Transitional Care Units (TCUs) within Skilled Nursing Facilities. Also, potential opportunity for Medical Director role of a TCU
On-call every other weekend for the first year and the every third week
ER call twice a month at a designated Hospital
Teaching opportunities with Residents and Med Students
