Easterseals UCP is excited to announce the start of a new primary care service to be part of an integrated care program with our Behavioral Health and Intellectual and Developmental Disabilities (IDD) service lines. In response to the transformation of North Carolina’s Medicaid program to managed care as well as the industry shift toward value-based reimbursement and whole-person care, Easterseals UCP is developing an integrated care model across the state, consisting of primary care and behavioral health providers, care managers, and IDD specialists. The program will be modeled after research on integrated care conducted by the AIMS Center at the University of Washington. Substantial research has shown that integrated care can lead to substantial improvements in clinical outcomes and cost reductions for persons with co-occurring chronic physical and behavioral health conditions.

Easterseals UCP is seeking a full-time, board-eligible/board-certified Family Medicine physician to join their clinic in Raleigh, NC. You will be part of a multi-disciplinary integrated care team, working closely with care managers and behavioral health providers to provide comprehensive, patient-centered care. As a member of our team, you will be providing full-spectrum, direct primary medical care to adults and children of all ages with intellectual and developmental disabilities and/or behavioral health diagnoses as well as their family members and other general population patients. Our team-based approach to patient care means you are part of a care team that is committed to community health and working with underserved populations in a highly collaborative outpatient setting.

Skills
Requires MD or DO from an accredited program with board eligibility or board certification in Family Medicine.
