Start ASAP Long Term or Perm
Two locations but willing to be flexible if provider only wants to cover at one location
M-F. 8:00 am to 5:00 pm 40hrs/week - will consider part time/flexible -no holidays, weekends or call
Average 12-18 patients/day
Non-Profit FQHC
Outpatient on-site care 
Routine and crisis adult and pediatric care as assigned.
Scope of practice includes evaluation, diagnosis, medication assessments and adjustment, patient education, and treatment planning.

REQUIREMENTS:
CT State License
DEA & State CS
BLS
PMHNP Certified
