A leader in preventive, value-based care we are seeking physicians and clinicians who appreciate senior patients, excel in a collaborative environment, and strive for positive outcomes for their patients. The group's focus is on outcomes, not on volume of patients.

Details:

• Physician run organization with peer collaboration
• New patients are scheduled at 40 minute intervals and follow ups are 20 minutes
• On average the daily schedule is less than 20 patients per day
• Balanced work life; Clinic Hours 7:30 - 5  Monday - Friday
• Practice Call - Phone call only 1 week every 6 months
• Exclusive focus on complex senior patients
• Small panel size is:  400-500 patients
