Description
Location: Dubuque, IA
Reason for Locum coverage: Vacations
Dates: July 11-15, Oct 19-21, Dec 19-30, 2022
Shifts/Hours: 8-430p every day, ½ hour for lunch
Call: None
Rate: Hourly/Daily, expenses paid
License: Preferred active, IA-open to CV’s who need license
Provider Needed: Locum PM&R
Certs: BC-PM&R, BLS, ACLS(preferred), Covid Vaccine,
Cred: 4 weeks
Census: 9 ppd
Coverage: Solo-HOSP there to help as needed.
Procedures: Required procedures: discharge summary/med rec, weekly care plan documentation, pre-admission screening review and documentation per CMS guidelines, clinical staffing/care plan meetings, PAPE/H&P, disability/impairment assessment, prosthetics and orthotics
Other info: Adult and Geri population. 9 bed IP unit. HOSP assists as needed. Unit staffed with 2 nurses, 1 CAN and 1 intake coordinator. EMR=Cerner-Powerchart. Level 3 trauma
