A healthcare facility in the Bronx is actively seeking a patient-oriented and dedicated Primary Care Provider (NP / FNP / ANP / PA) to join their staff.
About the Opportunity
Setting: Outpatient
Schedule: Monday to Friday, with rotating Saturdays
Shifts: 3morning/daytime and 2 evening shifts per week
Responsibilities
The Primary Care Provider (NP / FNP / ANP / PA) will be responsible for:
Screening, diagnoses, and treatment of patients
Making appropriate internal and external referrals
Documenting care promptly and following charting protocols
Ensuring appropriate follow-up for all aspects of ordered care
Qualifications
New York State Certification/Licensure according to degree
Current New York State Registration
DEA Controlled Substances Registration Certificate
CPR Certification
Board certification or eligible in Family Medicine, Internal Medicine or Infectious Disease for MDs/DOs
Ability to work effectively with diverse populations
Commitment to community-based primary care
Ability to work within a team-based structure in a pod model
Desired skills
Bilingual (Spanish/English)
Experience serving LGBTQIA+ and HIV-positive patients
