Great opportunity in Northern Indiana, not far from Chicago, to join a community health network for a Family Practice Physician.

• Must see all ages in this outpatient setting.
• Mon-Fri 8a-5p with one late evening a week until 6 or 7p.
• No on call schedule.
• These clinics serve all multiple primary care services in an outpatient setting.
• You will be sole physician alongside 2 NP's.
• Must be COVID vaccinated
