About the Facility - STAR (Stabilization Training Assistance Reintegration)
The State-Operated Stabilization Training Assistance Reintegration (STAR) homes are committed to affording each consumer a safe and nurturing environment where opportunities for growth, realization, self-expression, and goal achievement is celebrated. The homes strive to empowering individuals to be self-reliant as their skills, strengths, perseverance, and abilities allow so they can transition from crisis stabilization services to long-term community living. STAR provides services to adolescents (7-17 years old) and adults (18 years of age and older) with developmental disabilities receiving regional center services and who are in need of time-limited crisis stabilization services for up to 13 months. STAR homes provide person-centered support and crisis stabilization to the individuals being served, so that they can successfully transition to a community living setting.

Facility Address: This is a remote position; facilities are located in Vacaville CA, Springville/Porterville CA, and Costa Mesa CA.

RESPONSIBILITIES (listed are included but not limited to)
  • Of the three facilities that this position would be attending to virtually, STAR Homes serve individuals of the following ages: 7-12 years old, 13-17 years old, and 18 years old and older.
  • Responsibility for initial consultation: Assess patients in a timely fashion (based on facility policies, and depending on the individual's medical and psychiatric stability, recent and previous medical and psychiatric history, presence of significant or previously unidentified medical and psychiatric conditions).
  • Comprehensive initial history of a physical exam, which will include: a) screening for vision and hearing; b) any laboratory test, as determined necessary; c) health care maintenance.
  • Seek, provide, and analyze needed information regarding a patient's current status, recent history, and medications and treatments, to enable safe, effective continuing care and appropriate regulatory compliance.
  • For a patient who is to be transferred to the care of another health care practitioner, continue to provide all necessary medical care and services pending transfer until another physician has accepted responsibility for the patient.
  • Support patient discharges and transfers: Upon request, provide whatever documentation or other information that may be needed at the time of transfer to enable care continuity at a receiving facility and to allow the facility to meet its legal, regulatory, and clinical responsibilities for a discharged individual.
  • Make periodic, pertinent follow-up of patients: At minimum, virtual or phone check-in with each individual and assigned STAR Registered Nurse at STAR once a week.
  • Provide continuing care with individual's primary care physician and other medical professionals.
  • Provide continuing supervision of patient's health management.
  • Follows up on patients in a timely fashion, based on facility policies, consistent with applicable state and federal regulations, depending on the patients' medical and psychiatric stability, recent and previous medical and psychiatric history, presence of significant or previously unidentified medical and psychiatric conditions, or problems that require follow up.
  • Respond to issues requiring a physician's expertise, including the patient's current condition, the status of any acute episodes of illness since the last visit, test results, other actual or high-risk potential psychiatric problems that are affecting the individual's functional, physical, or cognitive status, and staff, patient or family questions regarding the individuals care and treatments.
  • Provide appropriate care to patients: perform accurate, timely relevant psychiatric assessments.
  • Properly define and describe patients' symptoms and problems, clarify and verify diagnoses, relate diagnoses to patient problems, and help establish a realistic prognosis and care goals.
  • In consultation with the facility's staff, determine appropriate services and programs for a patient, consistent with diagnoses, conditions, prognosis, and patient wishes focusing on helping the patients attain their highest practicable level of functioning in the least restrictive environment.
  • Respond in an appropriate time frame to emergency and routine notification, to enable the facility to meet its clinical and regulatory obligations.
  • Respond to notification of laboratory and other diagnostic test results in a timely manner, considering the patient's condition and the clinical significance of the results.

REQUIREMENTS
  • Board certification is preferred, but board eligibility is acceptable by the American Board of Internal Medicine (ABIM), American Board of Medical Specialties (ABMS), or the American Board of Family Medicine (ABFM).
  • Can be an MD General Practitioner or MD Family Medicine.
  • Must have five years' experience within their specific field.
  • Must have current, full, unrestricted license in California.
  • Must have current certification in Basic Life Support as certified by the American Heart Association.
  • Experience with patients that are developmentally disabled preferred but not required.
