SPECIALTY: FP/IM Outpatient LOCATION: Eureka, CA JOB TYPE: Locum Tenens JOB NUMBER: 46284 Details: I am looking for an IM or FP Physician to start in Eureka, CA for a locum’s opportunity. Travel and lodging needs can be covered. A decision is typically made within 7-14 days. Monday- Friday, 8a- 5p 18 PPD Mix of peds and adults EMR: EPIC Requirements: Active CA state license Will accept MD and DO BC and BE will be considered (if truly BC- 5 years or less of residency) Active DEA needed before starting. The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Initial Covid-19 Vaccine - Booster not required.
