  • Tuesday 7am to Tuesday 7am
  • Hospital Call Coverage
  • 1:5 Call Rotation
• Clinical Information:
  • EMR: EPIC
  • Patient Volume 5-10 Per Day
  • Procedures: colonoscopies and EGD
• Required:
  • Board Certified
  • Active IN License
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • CV must be in Month / Year format
  • Any gaps greater than 30 days will require an explanation on the CV
  • Active DEA
  • DEA must remain in state to maintain privileges.? Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
  • Current Flu Vaccine
  • Must be fully vaccinated for Covid 19
    • Exemptions only allowed for religious or medical reasons, once approved by the facility.
