Details of the Opportunity:
Under the general direction of the Chief Clinical Officer (CCO) and the direct supervision of the Medical Director, the physician will provide continuing and comprehensive health care for the individual and family across all ages and genders.
Within the context of Patient Centered Medical Home (PCMH) policies and procedures and within standards of care for each respective specialty (Family Medicine, Internal Medicine, Pediatrics, Psychiatry, Obstetrics and Gynecology), the clinician will exercise individual initiative and independent action to ensure high quality and comprehensive primary care.
The two central tenets of PCMH lie in team-based care and care coordination.
The physician is expected to function as an integral part of the Primary Care Team (PCT), as a leader and an engaged team member.

Requirements:
Ability to work effectively both independently and as part of a team.
Proven ability to work in a fast-paced environment, with excellent time management skills.
Strong communication and interpersonal skills, with commitment to establishing a safe and respectful environment for team members.
Ability to work in a culturally diverse environment.
High level of commitment to continuous improvement in the quest for comprehensive and quality health care.
Demonstrated interpersonal relationship skills.
Fluent in Spanish and/or Portuguese a plus.
Competency in working with Microsoft Office Suite and Epic is a plus.
Reliable transportation and the ability to travel between sites as needed is required.
