DESCRIPTION: I’m looking for a Primary Care physician to provide coverage in Florida. This is an outpatient clinic for company employees and their families. The patient population 2 and up. Will consider Internal Medicine doctors if experienced/comfortable w/age 2+.  Credentialing is FAST, approx 1 week. Please call if you are looking for PRN shifts

St. Augustine, FL:. Ages 2+, dispensing, Fingerprints req'd. DEA/Dispensing Lic not req'd
  6/30 8a-5p

Ponte Vedra, FL:  Ages 2+, dispensing, Must be ok with first treatment of injury. Fingerprints Required
  6/23 8a-5p, 7/14 8a-5p
  7/3 8a-5p
  7/6-14 Mon-Fri: M/Tu/W/F 6:30a-3:30p, Th 10a-7p

Melbourne, FL: Ages 6+
  6/22 6:30a-5p

Boca Raton, FL: Single provider clinic-no MA, may need to draw blood, No DEA req'd
  7/6 8a-4:30p


REQUIREMENTS: 
·        FL state license; active, unrestricted
·        BC/BE Family Medicine or Internal Medicine
·        Must be able to see ages 2+
·        2 years’ experience
