Schedule
- Monday-Friday

- 8am-5pm
Clinical Details
- Patients per day: 15-20 outpatient
- All ages mainly adult
- Outpatient clinic
Requirements
- Board Certified
- Active AZ License
- Active DEA
- BLS
- Minimum of 3 years experience
- Must adhere to Covid Vaccination Policy
