Description
Dates of Coverage: 7/03-9/29/2023
Schedule: 8am - 5pm Monday-Friday
Scope of Work: Diagnose and treat work-related injuries. Evaluate and treat urgent care patients on a walk-in basis.
Requirements: BC, BLS, DEA, active and unrestricted IN license, MUST have DOT cert, and Covid vaccination.
