Requirements
- Completed Masters Degree Level Nurse Practitioner Program or Physician Assistant Program from an accredited college/university
- Active unrestricted applicable licensure
- CPR Certification
- NPI & DEA
- Current National Board Certification
- BLS-Certification
- DOT Certified
- Prefer OCC Health Experience
- Ideal candidate will have at least three (3) years primary care practice experience, the ability to work independently, and experience working with and Electronic Medical Record.
- Provider should be comfortable seeing patients ages 2 and up
