A healthcare network in New York City is currently seeking a new, licensed  Nurse Practitioner (NP) and/or Physician Assistant (PA) for a promising opportunity with their Primary Care team at one of their juvenile centers in the Bronx. In this role, the Primary Care NP / PA will be responsible for providing medical care and promote health education our patients and perform administrative duties.   

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 10am to 6pm (Mon, Weds & Thurs), 11am to 7pm (Tues), and 8:30am to 4:30pm (Fri)
  • Patient Population: Teenagers (males mostly)

Responsibilities:

The Primary Care NP / PA will:
  • Provide medical care to adolescents keeping safety & security measures paramount
  • Take relevant patient history; identify patient needs and make diagnoses
  • Develop and implements treatment plans using diagnostic data
  • Perform complete well-care examinations including physical examinations
  • Write prescriptions and inform patients about medications prescribed, garnering parent/guardian consents, when needed
  • Complete medical forms and documentation related to patient care
  • Make and tracks all referrals to other agencies and providers, with follow-up care provided
  • Assist with phlebotomy, as needed, to ensure smooth patient flow
  • Administer immunizations and distribute medication to patients
  • Provide patient advocacy and participate in patient outreach, as appropriate for the site
  • Perform other duties, as needed
Qualifications:
  • 2+ years of experience
  • Bachelor's Degree
  • Active NYS licensed Nurse Practitioner (NP) and/or Physician Assistant (PA)
  • Experience in one of the following practice areas: Acute Care, College Health, Community Health, Family Health, Holistic Care, Pediatrics, Psychiatry, School Health, and/or Women's Health
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Master's Degree
  • Bilingual (English and Spanish)
  • Primary Care background
