    • Call as needed for Cath Lab / Office

• Clinical Information:
  • EMR= EPIC
  • Support Staff: Mix of RNs and MAs
  • 6 Physicians and 3 APPs
  • Will work in Inpatient and Outpatient
  • Special procedures: Interventional
• Required:
  • Interventional Cardiology skills required &All typical procedures necessary
  • Must be Board Certified
  • Must have IN license to be considered and presented.
  • CV must bein Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • Any gaps greater than 30 days will require an explanation on the CV
  • DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
  • Current Flu Vaccine
  • Must be fully vaccinated for Covid 19
    • Exemptions only allowed for religious or medical reasons, once approved by the facility.
