Description
Work Dates Needed: November 6 - ongoing
Preferred Schedule: M-F 8a-5p or 4 10s
Worksite Setting: Clinic
Scope of Work: Mix of peds/adults. Peds not required. 18 PPD, minimum is 18 PPD. Includes some Chronic Pain Management
Licenses, Certifications, Requirements: Active CA license, BLS, PALS, IM accepted.
The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Covid-19 Vaccine no longer required

Pay Rate: $145/hr
