NP / Nurse Practitioner / New York / Permanent / Family Nurse Practitioner (FNP) Job

A health services organization in New York City is actively seeking a licensed Family Nurse Practitioner (FNP) to join one of their federally qualified health centers (FQHCs) in Brooklyn. In this role, the Family Nurse Practitioner (FNP) will be responsible for providing high quality medical care to Adult & Pediatric patients, including preventive activities such as screening examinations, comprehensive health assessments, diagnosis and treatment of minor, acute and chronic medical conditions.

Responsibilities:

• Formulate diagnosis and institutes treatment based on objective data gathered
• Make referrals, as indicated (i.e., specialty clinics)
• Participate in regular chart reviews and other activities
• Promote and maintain high quality medical care for the patients of the satellite health center
• Coordinate patient care with other clinicians
• Perform all procedures and duties applicable to private practice in an office setting
• Participate in telephone on-call schedule, as needed
• Perform other duties, as needed

• 2+ years of Clinical experience as a Family Nurse Practitioner (FNP)
• Successful completion of an accredited Nurse Practitioner program
• NYS Family Nurse Practitioner (FNP) license

• Multilingual
• Community Health Center setting
