Description
A healthcare organization in New Jersey is currently seeking a licensed Nurse Practitioner (NP) for a promising opportunity with their Hospice & Palliative team.
Responsibilities:
The Hospice & Palliative NP will:
- Discuss advanced care planning and goals of care with patients and families
- Perform bedside palliative care patient/family evaluation
- Assist with diagnosis, treatment, and management of acute and chronic health conditions
- Order, perform, and interpret laboratory and radiology tests within scope of professional practice
- Prescribe medications including controlled substances to the extent delegated and licensed
- Order treatments and durable medical equipment as indicated
- Perform other therapeutic or corrective measures as indicated, including urgent care
- Assist in all facets of care coordination for palliative care referrals
- Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
- Assist in development of clinical practice guidelines/standards in support of quality palliative care
- Perform other duties, as needed
Qualifications:
- 5+ years of Clinical experience
- Graduate of a NYS approved Nurse Practitioner (NP) program
- NYS Nurse Practitioner (NP) license
- Certified in Hospice/Palliative Care
Desired Skills:
- Experience in a Geriatric, Hospice, and/or Intensive Care setting
