Schedule: Mon-Fri 8am-4:30pm

Providers Typical Day:

• On site supervision for compliance, checking SRS's, on treatment visits, occasional follow up or consult
• Cases - External beam treatments, OTV's and follow up procedures required
• 25-30 ppd

Requirements:

• Illinois License Required
• Minimum 1 Year Experience
• DEA Required
• BLS Certification
• CSR License
