Description
A health services network in Oregon is looking to add a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their team in a Remote role.

Responsibilities:

The Adult / Family Home Care NP / PA will:
  • Complete in-home and/or virtual comprehensive health evaluations
  • Capture an accurate picture of the member's health, including all current, chronic and historical conditions
  • Identify gaps in care and assisting to develop a plan to manage their health
  • Uncover clinical and social determinants that contribute to the management of the member’s health
  • Perform and interpret various lab and screening tests
  • Analyze potential contraindications regarding medications and interactions with over the counter drugs and supplements
  • Determine diagnoses based on supportive evidence and provide additional assessments and determinations of health issues
  • Evaluate how the assessment findings impact the member’s health
  • Identify risk factors that may exist and how this may impact their current health and need for further evaluation with referral to the health plan’s case management, primary care provider, and/or notifying adult protective services
  • Perform other duties, as needed
Qualifications:
  • Graduate of a College of Medicine or an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
  • Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • Board Certification
  • Completed at least 1 year of continuous Post-Graduate Training in a single accredited Internship, Residency or Fellowship program
  • Working knowledge of Google Suite (Google sheets, Google docs, Google slides, Google forms) and various computer software programs 
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Bi/Multilingual 
  • Adult, Family Medicine, and/or Geriatric background
  • Experience in Concurrent Review, Risk Management, and/or Utilization Review
  • Experience performing chart audits and educating peers on best practices
