Description
A healthcare company in New York City is actively seeking a licensed Nurse Practitioner (NP) to work between two of their nursing sites in Brooklyn.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: Flexible
- Setting: Nursing Home / Managed Long Term Care Facility
- Patient Population: Geriatric
- Patient Volume: Small caseload at each nursing home
Responsibilities:
The Nurse Practitioner (NP) will:
- Manage members with acute change in condition
- Review and manage pharmacotherapy of each member
- Prescribe appropriate medications, including controlled substances
- Identify, treat, and document changes in condition
- Order, review, and/or follow-up with treatments and diagnostic testing
- Complete timely health risk assessments
- Complete routine monthly comprehensive assessments
- Perform other duties, as needed
Qualifications:
- 1+ year of recent Clinical experience with the Elderly population
- Master's Degree
- NYS Nurse Practitioner (NP) license
- DEA registration
- Previous experience in I-SNP, HMO
- Working knowledge of Clinical Documentation
- Solid assessment, clinical, and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
