NP / Family Practice / Connecticut / Permanent / Primary Care NP / PA Job

A health services organization in Connecticut is currently seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) for a great opportunity with their Primary Care team in New Haven County.

Responsibilities:

The Primary Care NP / PA will be responsible for:

• Examine, obtain medical history and treat acute and chronic illnesses
• Order, perform and interpret diagnostic tests
• Create treatment plans, and involve patients and family members in those plans
• Initiate appropriate specialty referrals
• Collaborate on improvement in care delivery and service with management, other Physicians and staff
• Work with the Care team to close care gaps and improve quality measures
• Work with practice management to ensure compliance with billing and coding requirements
• Perform other duties, as needed

Qualifications:

• 1+ year of Primary Care experience
• Graduation from accredited APRN or PA program
• CT Nurse Practitioner (NP) and/or Physician Assistant (PA) license
• DEA registration and Controlled Substance certification
• Solid assessment, clinical, and documentation skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
