No Providers who worked for Envision within past 12 months and that live within 75 miles of the site No Providers who worked for this site or an affiliated site in the past 24 months No Providers that worked at this site or affiliated site that is within 75 miles of this site New Candidates that have never worked for Envision must live outside the 75 mile radius of this site **Please note that a call interview/Zoom Meeting with the Site Medical Director (SMD)/Clinical Leadership at the site will be required. Contact phone number and best time to contact over the next two weeks must be included with the presentation. Not including this information will delay the presentation from being presented. YOUR CLINICIAN WILL HAVE THE CHANCE, IF APPROVED, ON CLINICAL CALL TO ASK ANY ADDITIONAL QUESTIONS THAT ARE NOT INDICATED /ANSWERED ON THIS JOB ORDER. *** Experienced CRNA needed in *Gainesville, FL!!! Credentialing Time Frame: 90 days Case Types: General, Ortho, GI-Endo, Eyes, Healthy Peds, Urology, Robotics, OB/GYN, Bariatrics, Vascular, cardiac ablations, interventional radiology, neuro-interventional radiology, neuro, bronchoscopy. No trauma and cardiac. Avg Monthly Cases: 1,800 Avg Monthly OB Delivery: 325 EMR: Meditech Care Team Model: 25 CRNAs; 19 MDs Shifts: Preferred: 5-8s; 24 hour OB; Also available: 12s, 10s, 14s, 16s. Call shifts: Weekday beeper 3:30p-7a; Weekend call: 7a-7a 24 hour (Preferred to take 1 call shift per month) MUST BE WILLING TO TAKE CALL!!!
