A community services agency in New York City is looking to add a new Case Planner to join their growing Foster Care program in Brooklyn. In this role, the Case Planner will be responsible for providing permanency planning to expedite the safe discharge of children from the foster care system. 

About the Opportunity:
  • Start Date: ASAP
  • Assignment Length: Ongoing (Contract to Hire) 
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm or 9:30am to 5:30pm or 10am to 6pm
  • EMR: Electronic State System Connections
  • Patient Population: Foster Care (Ages 0 to 21)
Responsibilities

The Case Planner will:
  • Provide services for children, biological, and foster parents or planning resources
  • Perform both field and office work in handling a caseload for the agency
  • Gather information and providing services to meet clients' needs, focusing on permanency planning
  • Counsel individuals and transmit information on cases through the completion of agency and governmental documents
  • Investigate cases referred to the agency to determine need and proper method of care
  • Interview the individual, the family, community and governmental agency personnel, and any other resource person who might provide information necessary for a proper psycho-social diagnostic evaluation and the preparation of a treatment plan
  • Work with public welfare agencies, the courts, and others to assist individuals with financial, legal, medical, and personal problems
  • Perform other duties, as needed
Qualifications:
  • Bachelor's Degree in Human Services, Psychology Sociology and/or Social Work-related field
  • Good knowledge of the principles and practices of Social Casework
  • Good knowledge of the laws, rules, and regulations governing the care and treatment of children
  • Experience related to services to families and children
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Experience with Medical and/or Child Welfare
