A recognized healthcare network on Long Island is looking to add a licensed Psychiatric Nurse Practitioner (NP) to join their growing Assertive Community Treatment (ACT) team in Suffolk County. In this role, the Psychiatric Nurse Practitioner will be responsible for providing psychiatric assessments, medical assessment and treatment to ACT recipients and participates on a multi-disciplinary team. 

About the Opportunity:

  • Start Date: ASAP
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm
  • Setting: Assertive Community Treatment (ACT) team
  • Caseload: 10 to 15
Responsibilities:

The Psych NP will:
  • Provide direct, face-to-face and/or collateral services to recipients in the community, office or as directed by the recipient
  • Monitor the ACT recipients’ psychiatric conditions and delivery of medications and observes any non-psychiatric conditions
  • Provide services and treatment to recipients from diverse backgrounds in the community (off-site) as needed based on recipients and/or family preferences
  • Perform initial and periodic (as needed) psychiatric evaluations
  • Assess the need for psychotropic medication and assumes responsibility for its prescription and regular monitoring
  • Identify behaviors that interfere with treatment and recovery and conduct suicide and violence risk assessments to decide if an increased level of care is needed by increased contacts by the ACT Team
  • Develop, implement, evaluate, and/or modify person-centered goals to foster quality of care and achieve the best possible outcomes.
  • Frequently attend daily ACT team meetings and participates in regular treatment team meetings or case conferences to ensure the continuum of care for ACT recipients while they are on the road to recovery
  • Act as a role model, mentor and contributes to staff training and professional development by imparting expertise on assessment, diagnosis psychodynamics, medications, and the provisions of services
  • Recognize emergency situations and take appropriate action
  • Perform other duties, as needed
Qualifications:
  • Bachelor's and/or Master's Degree
  • NYS Psychiatric Nurse Practitioner (NP) license
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
