Where: Eagan, MN
When: 2/1-4/25 Thurs/Fri 7a-4p with 1 hr of admin time and 1 hr unpaid lunch
Site Details: NP or PA Need 12 week contract,
Patient age range: 2yrs and up. Clinic setting: in person/virtual from site. Primary Care with in-person visits and ability to do video/telephone visits. Amount of patients generally seen: 10-12/day on average.
Visit lengths are 45min for physicals/pre-op/new patients, 30min follow up visits/procedures and 15min telephone/in-person visits for medication refills or acute issues.
Closed weekends and nights, no call coverage required.
On-site staffing model: 1MA/provider, 1 MA at the front desk and on-site RN manager. Additional Remote support: Virtual Primary Care for members 24/7, 365 if they can't get into the clinic, RN Case Management and Care Navigation to support referral scheduling and tracking.
The clinic also has Behavioral Health, Physical Therapy and Chiropractic Care.
Full lab with several point of care testing options as well as lab draws that are sent to Quest for processing.
On-site prescription dispensing through MDScripts.
Require at least 2yrs of Primary Care and Chronic Disease Management experience.
Candidate must hold an active DEA license and an unencumbered practice license.
EMR: Epic (experience is preferred but not required)
