Join the medical staff of a multi-site nonprofit FQHC/community health center that is innovative, patient-centered, and emphasizes quality, access and affordability for all. Integrated behavioral health professionals and care coordinators are embedded in their Patient-Centered Medical Homes. Use an integrated approach to provide safe, therapeutic care in a holistic and systemic way, seeking to provide overall care management and useful patient and family education. Physicians here provide direction and guidance to APPs and others, serving as a resource and mentor.
