Where: Spanish Fork, UT
Need: NP/PA
When: ASAP - 4/25 Thurs each week - this could be on-going for Thursdays and possibly more each week
Shift: 7am to 5:30 pm
Duties: Primary Care, Occ Med, Urgent Care

REQUIREMENTS:
Active Utah license and Utah Dispensing license
Active DEA
Experienced with ages 2+
