• Schedule: M-F 8:30a - 5p (Clinic - 1 Day no contact day), Call 8a handoff (weekends F/SA/Sun)
• Setting: Clinic, L&D/PP

• Duties: Clinic, L&D/PP
• Required Procedures: All standard maternal fetal medicine procedures in a clinic and hospital setting, Obstetrics
• EMR: EPIC
• Support Staff: Front Desk and Clinical Support Staff, including ultrasound on-site
• Reason for Coverage: Physician Resignation

Clinic Details

• Patients per Day: 15-20
• Day-to-Day in Clinic:
  • 8:00-4:30 (~45-60 m for lunch)
  • Consults + read-only US for MFM and generalist read
  • Inpatient US consult support
  • Occasional private delivery patients or nurse midwife backup
• Consultative Only? Consultative, some private OB (can discuss if this is an absolute no)

Credentialing Information

• Timeframe: 60 - 90 days
• Credentialing with Other Facilities: CHI Health Lakeside Hospital
• Billing Enrollment: No, this position is not providing services in lieu of an existing enrolled provider for a period of 60 days or less.
• Requirements: Clinic, Hospital, obstetric privileges
