DESCRIPTION: 

·        3/4/24- 6/7/24 Monday- Friday, 40 hours/week

·        Primary Care- with Occmed services no DOT or Workers Comp

·        Wellness program: Smoking Cessation, Weight, Diabetes, Nutrition, Stress Mgt

·        EMR: eClinicalWorks

REQUIRED: 

·        IL state license; active, unrestricted

·        Board Certified

·        Certifications- ACLS and BLS

COMPENSATION:

·        Competitive Market Rate (rate based on availability, experience, certifications, housing, and transportation needs, etc.)

·        Malpractice insurance provided

SCHEDULE:

·        3/4/24-6/7/24 Monday- Friday, 40 hours/week.  M/Tu 6a-3p, W/Th 11a-7p, F 6a-12p
