Physician / Family Practice / Minnesota / Locum or Permanent / Primary Care Physician (MD/DO)- Locum Tenens Job

• Work Dates Needed: Monday - Friday (flexibility on 3-5 days/week) starting asap 
• Preferred Schedule: 8am-5pm with 1 hour lunch break, Tuesday hours: 11am-7pm 
• Worksite Setting: Outpatient/FQHC facility. 
• Scope of Work: Primary Care Visits. Routine Visits for well and illness and chronic disease management. All age ranges. Familiar with integrated health model and needs of immigrant families. Familiar with providing care with use of interpreter. 18-22 patients per day. 
• Licenses, Certifications, Requirements: MN Medical License, BLS, DEA, clean background check, and OIG. Credentialing with Medicaid and Medicare will be required.
• EMR: OCHIN EPIC
• Pay Rate Preferred: $115-115/hr.
