NP / Nurse Practitioner / Minnesota / Locum or Permanent / Family Nurse Practitioner (FNP-C) needed for Locum Tenens Job

• Cross Country Locums is seeking out a Family Nurse Practitioner (FNP-C) for a contract assignment in Minneapolis MN
• Work Dates Needed: Monday - Friday (flexibility on 3-5 days/week) to start asap! Month to month coverage is dependent on good fit. 
• Preferred Schedule: 8am-5pm with 1 hour lunch break, Tuesday hours: 11am-7pm 
• Worksite Setting: Outpatient/FQHC facility. 
• Scope of Work: Primary Care Visits. Routine Visits for well and illness and chronic disease management. All age ranges. Familiar with integrated health model and needs of immigrant families. Familiar with providing care with use of interpreter. 18-22 patients per day. 
• Support Staff: MA, RN, Front Desk and Call Center
• Licenses, Certifications, Requirements: MN Medical License, BLS, DEA, clean background check, and OIG. Credentialing with Medicaid and Medicare 
• EMR: OCHIN EPIC *Must have some form of EPIC experience*
• Pay Rate Preferred: $75-80/hr.
