Preferred Schedule: 1 week/month Office hours 9A-430P

Worksite Setting: Inpatient and outpatient

Scope of Work: 

Patient Volume: 10-20

Support Staff: 3 MAs, Audiologist

Special skills/ procedures: General ENT

Licenses, Certifications, Requirements: IN licensed, IMLC accepted, DEA, Board Certified

EMR: Epic for hospital work and Modernizing Medicine for office use.
