Description Nurse Practitioner Post Acute Care Network Skilled Nursing Facilities Multiple Positions in Northern & Central Illinois The Advocate Medical Group (AMG) is a physician-led and governed medical group committed to delivering the best health outcomes. The Group is a division of the Advocate Health Care system, one of the nation’s top 10 health systems based on clinical performance. AMG is one of Illinois’ largest medical groups with more than 1,100 physicians and over 110 Nurse Practitioners and Physician Assistants. As part of the Post Acute Network, APNs manage a caseload of skilled nursing home patients in collaboration with Advocate physicians. Positions are available around the Advocate BroMenn Medical Center, Advocate Christ Medical Center, Advocate Condell Medical Center, Advocate Good Shepherd Hospital, Advocate Illinois Masonic, and Advocate Sherman Hospital.
